CNA Registration Form
Include payment with the registration form:
Money Order or Cashier’s Check, payable to Regional Testing Center (NO personal checks will be accepted)
If approved by CA Department of Public Health (CDPH), please send copy of approval letter (932 form).
Bring the CDPH approval letter (932 form) to the test site. A original or copy of the approval letter will be accepted.
Applications must be received a minimum of 20 business days prior to the examination date for registration.
Note: Saturday, Sunday and national holidays are not considered business days.
Registration is based upon space availability. If requested date is full, we will email or call you for the next available date.
Notification emails and/or letters will be sent to you confirming examination date and location.
Notification emails/letters are not required for testing, but your name must be on the roster to take the examination.
Exam times will be announced upon registration.
