The applicant submit an online application above. Information collected in the online application has similar content that is on the Initial Nurse Assistant Application (283B form signed by the RN in charge of their training in Blue Ink) or the Approval Letter from CDPH (932 form). |

Applicants must upload a completed and signed 283-B or CDPH 932 form with their test application. |

Applicants will declare if the registration is an initial exam or repeat exam #2 and/or #3 on the drop-down menu. |
